Incretins and DPP4 inhibitors.

Incretins are gut hormones which stimulate pancreas to produce insulin. Exenatide(twice daily) and Liraglutide(once daily) are the two incretins to be used as injections for the treatment of Type 2 DM. Both apart from blood glucose reduction will also inhibit post meal glucagon production and will also produce weight loss. GLP-1 is a the major target of therapy for the future. However GLP-1 is rapidly degraded by DPP-4(Dipeptidyl peptidase 4)

DPP-4 inhibitors can increase active GLP-1 pool in the blood. DPP4 inhibitors soon to be in the market are Vildagliptin and Sitagliptin. Both are oral drugs with minimal adverse effects and produce no weight gain.
